Has your organization faced one or more of these scenarios?

Recurring problems? Contain and correct …then don’t forget, analyze and prevent!

 Some problems are probably worth just correcting with a quick fix. For instance, if a seal goes out and it only costs $20, then just correct and track and trend. On the other hand, if hundreds fail, then the situation is probably worth analyzing. Problems that hurt your organization the most – in the form of repair, replace or rework costs, downtime, lost revenue, penalties or fines – these must be the top priority for formal corrective action.
 
Unless your approach includes implementing and verifying correction, containment, and more importantly corrective and preventive action, you will not prevent problems from recurring. And unless your approach is grounded in thorough root cause analysis like Apollo RCA™, you will not be ready and able to successfully complete the necessary steps that truly reduce risk, prevent recurrence and eliminate repeat issues.
